All deliveries and major gynecologic surgeries are performed at Baylor University Medical Center at Dallas. Minor procedures will be performed in office, or at Baylor SurgiCare Center located near the office.

If you are a new patient, please arrive 30 minutes prior to your appointment to fill out new patient information.

CANCELLATION

Please notify us 24 business hours prior to appointment to avoid  cancellation fee.

NEW PATIENT

  • If you are a new patient please fill out your patient portal prior to your visit.

    • Photo ID such as your Driver License

    • Physical copy of your Insurance Card

    • List of your medications

    • Form of payment (cash, credit card, or check)
